Information you may need
Prescriptions
Please remember to bring an accurate and updated list of medications or medication bottles with you to each visit with the provider. This list needs to include the medication name, dosage and directions. This will help to ensure accuracy when refilling medications.
If you are in need of a prescription refill, please contact your pharmacy directly. It is the patient’s responsibility to notify the pharmacy in a timely manner to ensure medication doses are not missed. The pharmacy will typically send an electronic request directly to the provider for refills. This is the most efficient in ensuring medications are filled accurately. It is a good idea to contact the pharmacy a week before the refill is needed. This gives them time to contact us if there are any issues or for our office to contact your insurance. If you use a mail order pharmacy, please request a refill 10 business days in advance. Please do not wait until you are completely out of your medication. If you need to contact our office for a refill, please allow 72 hours for the request to be processed. All medication request require approval from your medical provider. You will be notified if your provider requires you to be seen in office before a refill is authorized. Please plan ahead for refills during Holidays and vacations.
Controlled substances will not be filled over the phone. This will require an office visit. Patients requesting a new prescription or antibiotic require an office visit or telehealth visit to ensure accurate diagnosis and treatment.
Prescription medications require close monitoring by your healthcare provider to ensure its safety and effectiveness. Regular office visits are required for all of our patients. The interval will vary depending on the type of medication prescribed. Most medications for chronic conditions require that you see your provider every 6 months for maintenance. This includes patients on medications for blood pressure, high cholesterol, diabetes, antidepressants etc. All other patients must be seen at least once per year for your annual wellness. It is our goal to provide you enough refills to last until your next required office visit. If you request a refill and are overdue for required visit, the provider may agree to send in enough medication to last until we are able to schedule a visit with you (up to 4-week supply). It is the patient’s responsibility to schedule your next appointment in advance and with adequate time to receive a prescription refill. Repeated no shows or cancellations of your required visit, may result in a denial of refills until you are seen in office by your provider.
